Basically ever single person in my group gets the below form. They can fill it out at any point for any reason in order to give someone "kudos" for doing something great for them or someone else at work.

It's really a cool little project with just that little piece alone. I'm completely amazed at how into it everyone is (including myself). However, the actual form above isn't the best part. The best part is the interaction we have once someone is nominated.
I have a corkboard in my office. Each time someone is nominated to get a Kudos, we don't tell the person right away. Instead, the person who is the "nominator" comes into my office and posts the Kudos form on the cordboard. I think take a picture with the iphone and email it to the entire group. This allows me, who admittedly doesn't always get to visit with all of my team, to get more in touch with my team at a ton of levels. I get to visit with the nominator (is that a word?) and then send a congratulations to the person nominated. It has been incredible in just the two short weeks it has been around. Great idea - my husband's company does a similar recognition thing - but they would put all of the "kudos" into a drawing at the end of the month, and someone wins a $25 gift card from giftcards.com
